Please SMS me on 0402 213 686 saying you want to make an appointment. Please include the your full name (or the client’s full name if they are a family member), DOB, GP name (or referrer) and best contact email address in the SMS.

You will receive an appointment time and date via SMS and will then receive email information, the forms you need to complete and relevant questionnaires. These need to be completed before the first appointment – please bring everything with you on the first day.

I recommend you bring a folder or a pouch with you to store various handouts and treatment modules in.
